Acne scars can be a devastating condition, and they have been notoriously difficult to treat. It is almost impossible to remove completely, but we have many options to make the appearance of the scars much better.

There are different types of scars: some are shallow and gradual, some are deep - "ice-pick" scars - some are pigmented. Usually, it is the depth differential between the scar and surrounding skin that creates a light-shadow interface, and makes them prominent.

Most patients have a combination of different types, and are usually best treated by combination therapy. It can be a fairly long process for the patient, and may require patience on the part of both the patient and the physician.

Depending on type of the scar, there are different methods for choice. There are three basic types of acne scars: Small diameter, shallow scars (Type 1), Small diameter, deep scars (Type 2) and Large diameter, deep scars (Type 3).
Type 1 scars respond well to the Fraxel laser and can usually be addressed by a filler injection.

Type 2 scars (also referred to as "ice pick" scars) may be removed by punch excision or may be treated with the CROSS technique, in which a tiny droplet of a chemical peel acid is placed in each scar (the acid solution "dissolves" the scar; with healing the scar is filled in).

Type 3 scars are best treated with the Fractional CO2 laser. Some Type 3 scars can be effectively treated with injection of a dermal filler such as Restylane or Juvederm. 

Dermabrasion has been a gold standard for treating extensive multiple scars. It is being gradually replaced by fractional CO2 laser skin resurfacing. Acne scars on the face especially the chin and cheeks are best treated with the CO2 laser. Typically acne scars are deeper than other scars and require a sanding or abrasion type of treatment. Acne scars can cause red or brown discoloration and uneven skin contour. Usually multiple types of treatments are needed for maximum improvement.
