If you have a midwife and a reasonably sized baby, tearing is rare. Massaging the area and/or putting pressure on it as the mom is crowning can prevent tears. My midwife did this.

Most docs cut but its actually better to tear on your own in most cases. You can always start to prepare yourself before birth by doing massage and stretching that way have less of a chance in tearing.

As for hemis I didn't get any from pushing I got only one from my 2nd son because he was so big that the pressure from carrying him inside caused it.

I kinda disagree, tearing is more trauma and less neat, when they cut it is more controlled and one straight incision.... when you tear it goes in all directions and harder to do a neat stitch afterwards, it will look messy and the va jay jay will also look differentOuch

i was told that natural tears heal easier...it also depends on the degree of the tear. i didn't want the episiotomy (sp?) unless it was absolutely necessary.

i tore but only needed a few stitches b/c it was small, but i also pushed for only 20min and he was a small baby

hemorrhoids are the worst...i got them after delivery and i remember when he was 3 days old i kept crying b/c i needed to poop but it really hurt, so i didn't want to. stool softeners help a lot.

don't be afraid to push! there are a lot of things that can help, and if you get an ep., you wont even feel it LOL

you can talk to your ob about your concerns...i didn't have a mw for the same reason.

or go to babycenter.com and join a birth club. u can find a lot of good info there, once you wade through all the passive agressive mommywar bs LOL
