something is wrong if a 3-4 yr old child can't put those two things together.

As soon as they are able to learn my name. My son calls me mommy at home but calls me by my real name at my parents house. It does not bother me. If he thought it was bothering me he would always call me by my first name. He often calls his dad by his first name. Probably because I call my husbands name a thousand times a day. I'm annoying like that.

She has only slipped up twice since then lol!

I've heard of the scenario though but I think it's crazy for your kids not to know your name. I do agree with not letting kids call their parents by their first names .
Or any adult

I could understand it if you have zero control over your child and you know that when they find out they are going to torture you buy calling you by your first name all the time. It would be best not to tell them if they are going to bully you.

never..heard..of..this.

What exactly is the concern?

Knowing your parent's names and calling them by their names is entirely two different things.

Just because the child knows it doesn't mean they will use it when speaking to the parent.
