It honestly depends on her and how she feels about it.
One of my besties loved her "Big" nickname so much, she got it tatted on her. Then, again I know other women who would perish at the thought. If she introduces herself to people that way, then she seems fine with it.
And yeah...men give women THEE worst nicknames!

It doesn't seem like that big of a deal to me especially if she doesn't have a problem with it. I know a few females with big in front of their name, but it usually refers to height or age, although I know 1 or 2 that use it to refer to weight.

Nicknames are weird.
One of my former co-workers went by Big Tasty, and she was at least 300 lbs. She loves it. I called her just "Tasty" once and she flipped out 
It might be safer to call her Lil T or regular T lol

I wouldnt suggest switching it up and calling her Lil T now, because then she'll think you're trying to be funny!!
