That's true, especially in the case of low budget productions. But I'd say the majority of the time, the performers know what they will be doing before hand, because it's tied into the price. Anal, slapping, DP, name calling, facials, etc etc are all discussed in the price offering stage.

I remember looking at an interview of one of the girls that did ghetto gaggers, and she said they told her 'we wanna do this, this, that, this, that, and this' and she said yay or nay.
